The fertility experts at the U. S. Food and Drug Administration (FDA) are warning about the increasing use of Clomid and other infertility drugs to improve fertility and help women who do not ovulate, especially during the second half of a new year. (Photo: AP)

Clomid and other infertility drugs are the latest treatment of choice for low-androgen women with ovulatory dysfunction who do not conceive after six to 12 cycles of treatment. In one study, women who took Clomid for six cycles of treatment with a combination of an FSH (follicle stimulating hormone) and an FSH receptor agonist in conjunction with a gonadotropin shot were ovulating again in a double-blind study. In the new study, the researchers compared Clomid and a different fertility drug, namely, letrozole, in a double-blind randomized trial. The results showed Clomid improved the chances of pregnancy in the first two cycles of treatment, and a higher chance of live birth with clomiphene citrate (Follistim) and letrozole in the second cycle.

Clomid and FSH receptors are found on the pituitary gland, brain and other organs in humans. They work by stimulating the production of follicle stimulating hormone (FSH) and luteinizing hormone (LH) that then signals the ovaries to release an egg.

In the new study, the researchers compared Clomid and FSH receptors in 10 patients with ovulatory dysfunction who were treated with the drug for six cycles. The FSH receptors in these patients were not found in normal women. The researchers found that the FSH receptors are more likely to be in the blood than normal women, and that the risk of developing infertility in the first cycle was about half that of the first. In the second cycle, the FSH receptors were found in women who didn’t ovulate, and the risk was about 5 percent of the risk for the first cycle.

“Clomid and FSH are not only effective in the treatment of low- androgen levels in women who do not ovulate, they are also effective in the treatment of hypogonadism in hypogonadal women who don’t ovulate,” said Dr. Jennifer Gersbach, a urology specialist at the University of California, San Francisco (UCSF) and a clinical associate professor in the department of urology at UCSF. “These results are significant because Clomid, which has been available since 1967, was the first FDA-approved medication for the treatment of hypogonadism, and it has been the most widely used fertility drug for decades. It has been used to improve the chances of pregnancy in the first cycle and to increase the chances of live birth in the second and third cycles.”

“Clomid, as a gonadotropin injection, has been the first treatment for low-androgen women with ovulatory dysfunction who do not ovulate,” Gersbach said. “It has been used in the treatment of hypogonadism and in the treatment of infertility and hypogonadism in hypogonadal women who don’t ovulate, but not as a treatment for infertility.”

The new study showed that Clomid and FSH receptor agonist, letrozole, can improve the chance of pregnancy in the first three cycles of treatment with the drug, but it did not improve the chance of live birth with letrozole, Gersbach said.

The results were published Wednesday in the journal Annals of Internal Medicine, and they were published in the journal JAMA Internal Medicine.

In the study, the researchers looked at the data of patients with hypogonadism who took a single dose of clomid, letrozole, and an FSH receptor agonist for six cycles of treatment with a single dose of clomiphene citrate, the drug’s main ingredient.

They compared the risk of live birth with clomiphene citrate in women who didn’t ovulate or who ovulated. The study showed that the risk was about 5 percent of the risk for the first cycle of treatment, and it was about 2.5 percent of the risk for the first cycle.

“The study was done because it was a double-blind, placebo-controlled, randomized study and the results of the study showed that in addition to taking clomiphene citrate in a single dose, the study also included patients who didn’t ovulate. But the study did not show that taking letrozole in a single dose is safe,” Gersbach said.
